Annual Wellness Visits
Your Medicare Advantage Plan provides coverage for an Annual Wellness Visit (AWV). An AWV is a scheduled visit with your primary care physician (PCP) that creates or updates a personalized health plan for that year. The goal of this visit is to get educated and to prevent illness.
What to Bring
- A list of all your medications (including vitamins and supplements)
- Referrals and/or a list of doctors you have visited in the last 12-16 month
Costs
Annual Wellness Visits are free of charge for our members and do not require any copays or deductibles!
